Well in simple terms- on-page SEO refers to optimizing your own website. Factors like website design, website content, meta descriptions, title tags and so on. On the other hand, off-page SEO refers to the page ranking factors that occur off your website, like Social Media Engagement, backlinks, Blogging, blog commenting, Forum Submission, etc.
